Please complete the first form including any comments or ideas that have arisen during the mentoring process. If you do not pass this information forward there cannot be any improvement in your professional development. The second form is your personal copy of the feedback sheet. This is a useful reminder of any targets agreed and the dates involved. It is also useful for you to check if there has been any response to your identified needs. It could be added to your personal professional portfolio.
